Now Hiring! 2 Community Link Worker Positions

 Job Summary: 

The Cowichan Seniors Community Link Project is a Social Prescribing Program for Older Adults at Risk of Frailty that will be operated through the Cowichan Seniors Community Foundation. Community Link Workers are responsible for carrying out the day-to-day work of the Community Link Project, including assisting and maintaining client cases and reporting to the Executive Director/Project Manager (ED). The program will work with Older Adults at Risk of Frailty in the Cowichan Region to determine what services are available in community that support 'aging in place'.
Link workers will assist clients in two formats.   A caseload of long-term clients will be carried by each worker to meet the goals of the social prescriptions they have been given by health professionals, social workers and community agencies. Workers will also assist these clients to identify areas of individual need and work with them to access additional community resources. Secondly, a client base will be assisted on an ‘as needed’ referral basis to direct them to services in community.  These strategies will improve seniors’ quality of life, encourage active engagement in community and support 'aging in place'. Areas of support might include access to nutrition, counselling, physical fitness, community engagement, transportation services and/or medical information.

 

Job Responsibilities:

  • Work closely with the ED and other team members collaboratively to meet our goals
  • Build relationships with staff of the Primary Care Network, Island Health and other health related agencies
  • Explore and develop partnerships with other community agencies serving seniors
  • Maintain strong methods of organization including planning, program development and task facilitation
  • Establish a client caseload, as well as assist inquiring seniors to community resources
  •  Help develop programs for those in your caseload to identify Individual senior’s needs and link individuals to relevant resources
  • Participate in seniors network meetings
  • Ongoing evaluation of the effectiveness of the established program for the individual’s program
  •  Record keeping in accordance with the needs of the individuals, the team, the grant and the ED

 

Skills:

  • Degree in Social Work, Community Development or equivalent
  • Ability to work independently but also take direction from the Project Manager and work in community team environments
  • Ability to assess client needs and work with them to meet those ‘Action Plan’ goals
  • Ability to work closely with medical, social work, community development and others to develop strategies aimed at improving quality of life for seniors
  • Ability to maintain clear and concise records
  • Ability to identify and facilitate clients to resources
  • Ability to collaborate with and motivate clients
  • Strong written and oral communication skills
  • Strong working knowledge of Microsoft Office, including Excel and Publisher

Other:

  • Proof of valid flu shot
  • Current driver’s abstract
  • Undergo a Criminal Record Check
  • Minimum of three years experience working directly with clients in community
  • Ability to travel broadly in the Cowichan Region

These two positions will begin early February 2020 and run until June 2022. Position pays $18/hour at 20 hours per week. Mileage paid at a rate of $0.56 per kilometer.  Payment is made in lieu of vacation.
